Terms of Service
Effective date:
1. Acceptance and website use
By accessing earley-care.com, you agree to these Terms of Service. If you do not agree, do not use the website. You may use the website only for lawful purposes and may not interfere with its security, operation, content, or other users.
2. General information; no medical advice
Website content is provided for general educational and informational purposes. It is not medical advice, diagnosis, treatment, or a substitute for evaluation by a qualified healthcare professional.
Using the website, submitting a form, calling, emailing, or sending a text message does not by itself create a clinician-patient relationship. A relationship is established only through Earley HealthCare's clinical intake and acceptance process.
3. Emergencies and urgent needs
The website, general contact form, email, and clinic text line are not monitored for emergencies. If you are experiencing a medical emergency, call 911. If you are experiencing suicidal thoughts or a mental health crisis in the United States, call or text 988.
4. Services, licensure, and telehealth
Service availability depends on clinical appropriateness, provider availability, insurance or payment arrangements, licensure, and applicable law. Telehealth services may be provided only when the patient is physically located in a jurisdiction where the treating clinician is authorized to practice.
Nothing on the website guarantees acceptance as a patient, a specific diagnosis, a particular medication or treatment, insurance coverage, or any clinical outcome.
5. Appointments and secure communications
Use the practice's designated scheduling and secure communication methods for appointments and clinical information. Do not place detailed health information, medication lists, Social Security numbers, payment-card information, or urgent requests in a general website form or ordinary text message.
6. SMS text messaging program
If you provide consent, Earley HealthCare may send conversational and informational text messages, including replies to inquiries, appointment scheduling and reminders, care coordination, and account or administrative updates.
Message frequency varies. Message and data rates may apply. Mobile carriers are not liable for delayed or undelivered messages. Text delivery is not guaranteed and may be affected by your device, carrier, network, or number status.
Reply STOP to opt out. After you opt out, you may receive one confirmation message. Reply HELP for help, or call 318-600-3456. Opting out of text messages does not prevent you from receiving care; we may communicate through another available method.
Consent to receive text messages is optional and is not a condition of receiving care or purchasing any service. We do not send marketing or promotional messages under this patient-communications program.
Avoid sending detailed clinical information by text. SMS is not always encrypted and may be visible to anyone with access to your device or mobile account. Contact the practice to learn which secure communication options are available.
7. Privacy
Our Privacy Policy explains website and SMS information practices. Our Notice of Privacy Practices separately explains how protected health information may be used and disclosed under applicable healthcare privacy law.
8. Intellectual property and third-party services
Website content, branding, text, graphics, and design are owned by Earley HealthCare or used with permission and may not be copied or distributed without authorization, except as permitted by law.
Third-party links, scheduling tools, maps, and services are provided for convenience. Earley HealthCare does not control their availability, content, security, or privacy practices.
9. Disclaimer and limitation
To the extent permitted by law, the website is provided on an "as available" basis without warranties that it will be uninterrupted, error-free, or suitable for every purpose. Earley HealthCare is not responsible for losses caused by reliance on general website content, third-party services, network failures, or unauthorized access outside our reasonable control.
Nothing in these terms limits rights or responsibilities that cannot legally be limited, including obligations that apply to professional healthcare services.
10. Changes and contact
We may update these terms by posting a revised version with a new effective date. Continued website use after an update means you accept the revised terms to the extent permitted by law.
